Reablement Care Assistant for Senior Citizens - Community Lifeline

Reablement care, also referred to as intermediate care, is a type of short-term home care that may be required following a diagnosis or long hospital stay due to illness, surgery, accident, or stroke, for example. Reablement care is specifically designed to assist people in living the way they wish to and doing day-to-day tasks more independently after a designated period of care.

Reablement care refers to the support of a trained Community Lifeline caregiver who visits your home to help improve mobility and assist with daily activities and tasks, like getting dressed or preparing food. It can run for a maximum of six weeks or as little as two weeks. This care helps a person regain their skills so that they may live at home independently with little to no assistance.

An integrated care team is usually involved in providing reablement care and typically involves professionals from all areas of the NHS and the wider social team. This may include nurses, social workers, homecare workers, occupational therapists, physiotherapists, social workers, and doctors.
