Integrated Domiciliary Care (IDC) is where different organisations work together for the sole purpose of providing an individual with the very best support.

IDC aims to put the individual at the centre of their care plan with different professionals working seamlessly together to achieve the same outcomes.

Many different professionals in healthcare are involved in an integrated care plan. This is so that different services can work cohesively together to achieve the best outcome for individuals. They may come from the NHS, private health providers, and even the local council, such as a…

  • Social worker
  • Physiotherapist
  • Occupational therapist

  • Community mental health team

It is important to have fluidity between different services that someone may need in order for them to receive the most comprehensive and effective care, including but not limited to, physical, mental, and holistic care.

As a client’s needs change, so do their healthcare providers also change. For example, if someone needs to recover from a serious illness that has affected their mobility, they may need round-the-clock care from a carer, with additional help from a physiotherapist, and weekly check-ups with their GP. If the client gets better, their carer’s hours and physiotherapist may reduce, and their GP may request more regular check-ups.
